Medication review - the what and the how

Introduction 

In 2009 the Pharmaceutical Care Network Europe (PCNE) started to discuss a definition and terminology for medication reviews (MR) performed by pharmacists in ambulatory and clinical setting. The aim of the special interest group (SIG) is to bring experts together in order to create a set of guidelines for conducting medication review, including an algorithm for patient selection. At various meetings and workshops the PCNE definition and a terminology were developed. The development and agreements achieved during PCNE meetings from 2009 until 2014 will be presented focusing on the definition, different types, characteristics and examples of applied science of a medication review. The PCNE terminology takes into account that the source and type of available information defines the type of medication review and the drug-related problems that can be detected. Examples of best practice reflecting the different types from different countries care will demonstrate how to perform such a medication review. Illustrated by a patient case the different methodologies and steps of a medication review, software solutions and documentation forms will be presented including the evidence available regarding this service. The focus of this session organised by the CPS in cooperation with PCNE is on medication review as part of primary care.
